Exercise and GLP-1: How to Maximize Your Treatment Results

Exercise and GLP-1: How to Maximize Your Treatment Results

The scale stops moving and you do not know why. Understand why exercise is essential during GLP-1 treatment and how to track your progress the right way.

The scale has not moved in weeks. The dose is right, the meals are on track, yet the number on the display stays put. That scenario is more common than it sounds during GLP-1 treatment, and the answer often lies beyond the scale itself: it sits in how the body responds to weight loss. Start here with PeptPro and track your workouts and weight evolution over time.

GLP-1 medications drive weight loss by curbing appetite and slowing gastric emptying. Results show up, but body composition matters as much as the number on the scale. When physical activity enters the equation, the treatment gains an ally that helps more than the scale number suggests.

Why Exercise Makes a Difference During Treatment

A study published in the New England Journal of Medicine in 2021 followed 195 participants who used liraglutide combined with supervised exercise. After one year of treatment, the group that combined the medication with physical activity retained more lost weight than the medication-only group. When treatment was discontinued, that difference became even more apparent. The exercise group managed to hold on to most of its results; the other group regained weight at a faster pace.

Those numbers show that exercise is more than a supplement to treatment. It acts as something that helps sustain weight loss even when the medication is reduced or stopped. The WHO estimates that 43% of adults worldwide are overweight, and that regular physical activity is one of the most consistent factors in maintaining weight loss.

With PeptPro you log your workouts and track weight over time, which lets you see whether results are coming through as expected. By syncing with Apple Health, you have everything organized in one place to review before your next appointment.

Resistance or Cardio: What to Prioritize

When it comes to weight loss with GLP-1 medications, resistance training carries specific weight. A review published in 2024 in the journal Diabetes Care examined how incretin-based medications affect body composition. The data is clear: up to 40% of weight loss during treatment can be lean mass, not fat, when physical activity is not part of the routine.

That number deserves attention. Losing muscle mass is not just an aesthetic concern. Muscle is metabolically active, which means it burns calories even at rest. The less muscle you have, the slower your metabolism runs, and that creates a cycle that makes maintaining lost weight harder.

For people on GLP-1 treatment, the priority should be strength training at least twice a week. This does not mean lifting the heaviest weights in the gym. It means challenging your muscles progressively, whether with dumbbells, resistance bands, or even your own body weight. Free weights or elastic bands help preserve and even build lean mass during weight loss.

Cardio still has its place, but order matters. Jensen SBK et al. published a 2024 study in EClinicalMedicine showing that participants who maintained aerobic exercise and resistance training during GLP-1 receptor agonist use had better body composition after one year without active treatment. This means starting with resistance and then adding cardio produces better results than doing cardio alone.

What Happens When Weight Loss Is Too Fast

When you lose weight rapidly, the body has to decide what to sacrifice. It prioritizes energy and, when in doubt, cuts muscle before fat because muscle costs more energy to maintain. That process is a biological response: the body does not know you are pursuing healthy weight loss, it simply responds to the caloric deficit.

The problem is that when treatment ends or the dose is reduced, those who lost significant muscle face a slower metabolism and a greater tendency to regain weight. Exercise acts as a brake on that process. By challenging your muscles with resistance, you signal to the body that the lean mass is needed and should be preserved.

Keeping up with exercise during treatment is not about optimizing weight loss. It is about making sure the weight loss you achieve sticks, with less muscle lost and more fat removed.

    What to Measure Beyond Weight

The scale measures total weight, not composition. Two people at the same weight can have completely different ratios of muscle and fat. That is why tracking only the number on the scale gives an incomplete picture of progress.

Body measurements give more information about what is actually changing. Waist circumference, for instance, is an indicator of visceral fat that the scale does not capture. Strength and endurance also provide important signals. If you are lifting more weight or doing more repetitions than you were a month ago, your body is responding to training even if the scale has not moved.

PeptPro lets you log weight manually and follow your progress over weeks with simple charts. For Apple Health users, weight and physical activity data flow automatically into the app, making it easier to track without having to write everything down by hand.

How to Start Exercising During Treatment

For those beginning or returning to physical activity, the most important rule is to resist doing everything at once. Consistency beats intensity when the goal is to build a sustainable habit.

Walking thirty minutes a day already brings measurable benefits for most people. A 2024 review in Diabetes Care recommends that patients on GLP-1 treatment prioritize resistance exercise before adding cardio, precisely because of the role muscle plays in protecting body composition.

Start slow and increase load gradually as your body adapts. If the idea of going to a gym feels intimidating, try exercises at home using your body weight or resistance bands first. The important thing is building the habit and keeping it for at least a few weeks before ramping up intensity.

Hydration also deserves extra attention during exercise. People on GLP-1 treatment need to be particularly careful with water intake because the medication can affect thirst sensation. In PeptPro you can log your daily hydration, which helps maintain the habit of drinking water even when thirst does not signal clearly.

PeptPro brings together dose, meals, and workout history in one place. Instead of relying on memory or scattered spreadsheets, you open the app and everything is there. Start here.
