Most people on peptide-based treatments like semaglutide or tirzepatide are not drinking enough water. It is not a knowledge problem. The side effects of treatment confuse thirst signals, and water loss through the gastrointestinal tract is higher than expected. Understanding this relationship makes a real difference in outcomes.

What Happens to the Body During Treatment

GLP-1 agonists slow gastric emptying. Food stays in the stomach longer, and the digestive system operates differently than usual. For people on weekly injections, this effect is constant.

At the same time, many notice increased urination frequency, either through water loss via the kidneys or through diarrhea and gastrointestinal discomfort that causes the body to shed extra fluid.

The combination is straightforward: you lose more water than normal and may not feel thirsty with the same intensity. The body enters a kind of scarcity mode and reduces the urge to drink, even when it needs to.

Signs of Dehydration You Should Not Ignore

Some signs indicate your hydration is compromised. Dry mouth that does not go away even after drinking water. Fatigue that is disproportionate to the effort of your day. Headaches that appear in the late afternoon and disappear after sleeping.

Dizziness when standing up quickly from bed or a chair is an important signal. When blood volume decreases due to lack of water, blood pressure drops and the body takes time to compensate.

Dark urine with a strong smell means your kidneys are concentrating waste because there is not enough liquid to dilute it. On hot days or during exercise, this worsens quickly.

The Right Amount of Water Per Day

There is no single rule. Body weight, physical activity, climate, and diet determine how much you need. As a reference, a 70 kg person needs approximately 2.5 to 3 liters of water per day under normal conditions. With exercise or heat, that number increases.

A practical starting point is dividing body weight by 30. The result in liters is a reasonable estimate to begin with. A person weighing 75 kg, for example, starts from 2.5 liters daily.

Vegetables and fruits contribute water too. Cucumber, watermelon, orange, and lettuce have high water content and add to the total. Soups and broths help as well.

Electrolytes: What Sodium and Potassium Have to Do With This

Drinking water alone is not enough. When you sweat heavily or have diarrhea episodes, you lose not just water but also sodium, potassium, and magnesium. These minerals are responsible for muscle contraction, heart rhythm, and the acid-base balance of the blood.

Replacing them matters more than just drinking plain water. Coconut water is a natural option. Sugar-free sports drinks work for people who exercise. Powdered electrolyte supplements are another practical alternative.

If you experience leg cramps, muscle fatigue, or irregular heartbeat, it is worth talking to your doctor about electrolyte replacement.

How PeptPro Helps You Track

The PeptPro app lets you log water intake daily and follow patterns over weeks. By cross-referencing these data with your medication dose and events like diarrhea or excessive sweating, you can identify whether there is a relationship between what you take and how your body responds.

If nausea appears on days with low water intake, or if headaches arise when you forgot to drink for several hours, the app history shows that clearly. Download here.

You can also set personalized daily goals and receive reminders at times that fit your routine. The app keeps everything organized alongside your dose, symptoms, and weight progress, so you do not need to check different pages to understand what is happening.

Keeping hydration in check does not fix every side effect of treatment, but it removes a factor that worsens several of them. Start tomorrow with a realistic goal: note how much you drank today and add half a liter more tomorrow.
