Patients with hypothyroidism on levothyroxine often ask whether they can use GLP-1 medications. The short answer is yes, but monitoring is important.

What the research shows

The original concern came from animal studies where GLP-1 receptor agonists were associated with thyroid C-cell tumors in rodents. However, this has not been observed in humans at therapeutic doses. Current clinical guidelines state that GLP-1 medications can be used in people with hypothyroidism, but they should be monitored more closely.

The more practical concern is that GLP-1 can affect how levothyroxine is absorbed in your intestines. Because GLP-1 slows gastrointestinal motility, levothyroxine may not be absorbed as efficiently if taken at the same time.

Why timing matters

Levothyroxine is best absorbed in the small intestine when the stomach is relatively empty. Taking it with food or other medications can significantly reduce absorption. If GLP-1 slows stomach emptying, taking levothyroxine close to your GLP-1 injection or near meals can reduce how much of the hormone your body actually gets.

The practical recommendation is to take levothyroxine at least 30 to 60 minutes before your GLP-1 injection or food, and to be consistent about the timing every day.

What monitoring looks like

TSH should be checked before starting GLP-1 treatment and every 3 to 6 months during treatment. If your TSH rises, your levothyroxine dose may need to be increased.
