Retatrutide (LY-3437943) is the first medication in clinical development that activates three receptors at the same time: GLP-1, GIP, and glucagon. While GLP-1 and GIP handle appetite and blood sugar regulation, glucagon adds a layer that raises energy expenditure. That makes this approach structurally different from anything currently available. Eli Lilly developed the molecule with the explicit goal of hitting weight loss from every possible angle.

How the triple mechanism works is worth understanding in plain terms. GLP-1 sends satiety signals to the brain and improves how the body handles insulin. GIP amplifies insulin release after meals. Glucagon, the third piece, pushes the body to burn more calories at rest by promoting lipolysis, the breakdown of stored fat. The three pathways do not overlap, they complement each other. Studies published in Cell Metabolism by Coskun and colleagues in 2022 laid out this design rationale clearly.

The result of combining these actions is a reduction in calorie intake that outpaces what dual agonists can produce. Tirzepatide, for comparison, activates GLP-1 and GIP only. Retatrutide adds glucagon on top, which seems to account for the greater weight loss seen in early trials.

The phase 2 data, published in The Lancet by Jastreboff and colleagues in 2023 (trial NCT04881760), showed participants with obesity receiving weekly doses of up to 12 mg losing an average of 24% of their body weight over 48 weeks. That number exceeds the 20 to 22% range typically seen with tirzepatide in comparable timeframes. The trial involved adults without diabetes, and the effect scaled with dose.

No direct head-to-head trial between retatrutide and tirzepatide has been published yet, and retatrutide has not received regulatory approval. Still, the magnitude of the phase 2 results positions it as a candidate to lead the next wave of incretin-based therapies if the phase 3 data holds.

The side effect profile resembles that of other GLP-1 agonists. Gastrointestinal symptoms, nausea, vomiting, diarrhea, and constipation are the most commonly reported. Some participants experienced mild tachycardia and headache at certain dose levels. The long-term safety record is still being built through the ongoing phase 3 program.

The known class risks, including pancreatitis and gallbladder disease, have not been definitively linked to retatrutide in available data, but they remain considerations for the entire GLP-1 family.

The TRIUMPH phase 3 program is where things move next. TRIUMPH-1 (NCT05565716) is enrolling adults with obesity or overweight. TRIUMPH-3 (NCT05579951) focuses on severe obesity combined with established cardiovascular disease. There is also a substudy in type 2 diabetes. Primary results from the two pivotal trials are expected in April 2026. Until then, retatrutide remains an investigational drug.

What retatrutide represents is a shift in how obesity pharmacotherapy is being designed. Rather than targeting a single pathway, the approach looks at multiple metabolic levers simultaneously. If approved, it will offer an option for people who have not reached their goals with current therapies. Careful medical follow-up will be key to identifying which patients benefit most.
