Sarcopenia is the progressive loss of muscle mass and strength that occurs with aging, but it can be accelerated by prolonged calorie restriction and rapid weight loss. Understanding how GLP-1 treatment interacts with muscle preservation is not a technicality. It is central to whether the treatment works well long term.

Why GLP-1 Medications Affect Muscle Tissue

GLP-1 agonists work primarily by reducing appetite and slowing gastric emptying. When you eat significantly less over weeks and months, your body needs to source energy from somewhere. Glycogen stores are used first, then fat. If the calorie deficit is large enough or sustained long enough, the body starts breaking down muscle protein for amino acids.

The good news is that muscle loss associated with GLP-1 treatment can be minimized with consistent, practical steps.

The first is protein intake. General recommendations for people in active weight loss with resistance training range from 1.2 to 1.6 grams of protein per kilogram of body weight per day. For an 80-kilogram person, that is 96 to 128 grams distributed across meals. Food sources like chicken, fish, eggs, dairy, legumes, and whey protein supplements can help reach that target.

Resistance training is the second essential pillar. Weight training, resistance bands, or bodyweight exercises send a clear signal to the body that muscle tissue needs to be maintained. Two to three sessions per week targeting major muscle groups is sufficient. Daily training is not necessary. Recovery is part of the process.

Rate of weight loss matters. Losing 0.5 to 1 kilogram per week is generally considered sustainable for muscle preservation in most individuals. Faster rates of loss increase the proportion of weight coming from muscle.

Long-Term Maintenance

Preserving muscle during GLP-1 treatment is not only a short-term concern. Muscle is metabolically active, burning calories at rest. People who lose significant muscle mass during weight loss have a harder time maintaining their weight after stopping the medication, because their basal metabolic rate has dropped.
